Hiatal Hernia Test

Im wondering if I might have a hiatal hernia. I have been taking protonix for GERD for 8 weeks and my reflux has actually gotten worse. I have been avoiding all the foods youre not supposed to eat with GERD, but every thing causes me heartburn, even bread or plain crackers. In the past few days, Ive been having chest pains on my left side that feels like a burning, sore, heaviness, that is pretty constant. I have an appointment with a GI doc a week from nowadays to have an endoscopy. On a side note, Im 28 and 100 lbs so weight isn't an issue. Could this be a hiatal hernia or just severe GERD? Have you had chest pain or what are some other symptoms you've had? Also, what causes hernias? Thanks
